Housing Assistance Rental Transition
The HART program was developed to help adults with a serious mental illness find affordable housing in order to prevent homelessness. The program is primarily for persons who are being discharged from an inpatient placement and who have no place to live. The primary goal of the program is to create a bridge from the Housing Related Assistance Program to more long-term stabilized housing such as Section 8, Public Housing, or independent housing.
HOW DOES SOMEONE QUALIFY FOR THE HART VOUCHER PROGRAM?
- Applicants must be at least 18 yrs old and meet the financial guidelines of the program.
- Must have a diagnosed mental illness and a treatment plan or service plan with a goal of independent living.
- Applicants will need to have a Community Support Worker, or a person trained to act as your CS Worker, and be willing to work to achieve a goal of independent living.
- Must be receiving HHSS registered or authorized behavioral health services.
- Must be Nebraska resident.
THERE ARE TWO GROUPS OF PEOPLE CURRENTLY BEING SERVED BY THE HART VOUCHER PROGRAM
Priority One:
- Extremely Low Income adults being discharged from an inpatient mental health commitment
- Extremely Low Income adults who are eligible to move from a residential level of care to independent living to make room for a person being discharged from an inpatient mental health commitment
Priority Two:
- Consumers who are extremely low income and who are at risk of an inpatient mental health commitment, at least in part because of a lack of affordable housing.

HART Housing Success Story
A HART receipient participated in the program for one year establishing a good rental history. After which the person was able to go on and buy their own home through a program sponsored by housing authority. This person has not returned to the program.
